Privacy Policy
This policy explains what information WorkPlan, a trade name of HealthApply, collects through workplan.health, how we use it, and the choices you have. WorkPlan helps employers offer health benefits through a CHOICE Arrangement, formerly called ICHRA. This site is for business visitors.
Information you give us
When you send a request through the form, we collect your name, work email, company, state, number of employees, current coverage type, and the month your plan renews. You may also give us a phone number, notes, and the numbers from the budget sketch. Please do not include health information about any person.
The budget sketch runs in your browser. Nothing you type into it is sent to us unless you choose to send those numbers with a request.
If the form cannot send your request automatically, the page opens an email draft in your own email app. That message then travels through your email provider.
Information collected automatically
Our hosting provider records basic technical information when you visit, such as your IP address, browser type, device, and the pages requested. This helps keep the site secure and running. The site loads fonts from Google, which means Google receives your IP address. We do not currently use advertising cookies or analytics tools on this site.
How we use information
- To respond to your request and talk with you about your options
- To set up your account and the platform if you choose to move forward
- To answer questions and give support
- To keep the site secure, prevent misuse, and meet legal requirements
- To improve the site and our services
We do not sell your personal information, and we do not share it for advertising.
Who we share it with
- Service providers that run the site and our email. This includes our hosting provider, which also stores form requests, and our email provider.
- If you choose to move forward, the national licensed insurance agency assigned to your account, so it can do the enrollment work for your employees.
- Advisors such as lawyers and accountants, and authorities when the law requires it.
- A buyer or successor if WorkPlan is part of a sale or reorganization.
Security
We use reasonable safeguards, including encrypted connections to this site. No method of sending or storing information is completely secure.
Your choices and rights
You can ask to see, correct, or delete the information we hold about you, and you can ask us to stop contacting you, by emailing hello@workplan.health. Depending on where you live, you may have additional rights under state privacy laws. We will respond as the law requires.
Links to other sites
This site may link to other sites. We do not control them and are not responsible for their practices.
Children
This site is for business visitors and is not directed to anyone under 18. We do not knowingly collect information from children.
Changes to this policy
We may update this policy from time to time. The effective date above shows the latest version.
